Are AI Overviews Changing Google Ads for Physical Therapy Clinics?
Short answer: less than the headlines want you to believe — at least on the searches that actually bring you patients. AI Overviews (Google’s AI-generated answers at the top of the results) have genuinely reshaped search, and yes, ads now appear inside and around them. But for a local physical therapy clinic, the change is lopsided in a way that’s easy to miss if you only read the panic: Google pulled AI Overviews out of local “near me” searches and left them all over the informational ones. That single split decides what you should actually do.
Here’s the shape of it. The ready-to-book searches your ads live on — “physical therapy near me,” “[your town] physical therapy” — are back to plain local results and standard ads; AI Overviews have effectively vanished from them. The searches AI Overviews took over are the informational ones — “why does my knee hurt,” “exercises for a torn meniscus” — which were only ever bringing you blog traffic, not booked patients. So the real story isn’t “AI is eating your ads.” It’s that search has split into two lanes, and your clinic wins each of them a different way.
Here’s how AI Overviews and ads actually work now, which of your searches are affected, and what to change — and what not to.
What are AI Overviews, and where do ads fit?
AI Overviews are the AI-generated summaries Google places above the traditional results, pulling from several sources to answer a question directly. Google says they show mainly on “no one right answer” queries — complex or exploratory questions — and that ads can appear within or beside them only when there’s commercial intent and a relevant, quality ad to show. You don’t opt in: if you already run Google Ads — particularly Performance Max or the newer AI-driven “AI Max for Search” campaigns — you’re automatically eligible for these placements. There’s no separate “AI Overview campaign” to build.
The important part for a clinic isn’t the plumbing, though. It’s which of your searches trigger an AI Overview in the first place — because that has changed a lot, and not in the direction most people assume.
Do AI Overviews show up on the searches that bring you patients?
Mostly no — and that’s the good news. When AI Overviews first rolled out they appeared on nearly everything, including local “near me” searches. Then Google reversed course: provider-finding and “near me” healthcare searches, which had briefly shown AI summaries, now essentially never do. A search like “physical therapy near me” or “[your town] sports physio” hands back the same thing it always did — the local map pack, local listings, Google Business Profiles, and standard ads. No AI paragraph sitting on top.
That matters because those transactional, local searches are exactly where your Google Ads and your Business Profile turn browsers into booked patients. Google decided — sensibly — that when someone wants a provider near them, a list of real local options serves them better than a summary. So the part of your paid and local presence that does the heavy lifting is largely untouched. If anything, the local lane is cleaner than it was two years ago.
Where AI Overviews did take over — and what to do about it
The informational tier. Treatment, condition, and pain-related searches — “what helps a torn meniscus,” “is my back pain serious,” “stretches for sciatica” — now trigger an AI Overview almost every time, long and specific questions most of all. On those searches Google answers inside the summary, and far fewer people click through to any website beneath it.
But look at what those queries really are: research, not booking. They were never filling your schedule directly — they were top-of-funnel traffic to blog posts. What’s changed is how you win them. You no longer win an informational search by ranking a page and collecting the click; you win it by being one of the trusted sources the AI Overview cites in its answer. That’s earned through genuine authority — clear, credible, well-structured content on the conditions you treat — and it’s the very same work that gets your clinic surfaced in ChatGPT and other AI answers. Being cited is the new “ranking” for that lane, and it’s worth having: the clinics that get cited tend to pull more click-through, not less. How to become that source is its own subject — we cover it in getting found in AI search.
Can my clinic’s ads appear inside an AI Overview?
For now, largely no — and you shouldn’t build a plan around it. Google has kept regulated categories, healthcare among them, out of the ads that show within AI Overviews, and has only recently begun a small US test of healthcare ads in its fuller AI Mode. That mirrors what’s happening on ChatGPT, where health advertisers are gated too — we covered that here. None of this stops your ordinary Google Ads from running; they serve as normal on the local and transactional searches above. It just means the shiny new in-AI placement isn’t a lever a local clinic can pull yet — so it isn’t worth chasing.
So what should a PT clinic actually do?
Don’t panic-rebuild your strategy around a threat that skipped your booking queries. Concretely:
- Keep running Google Ads on local, transactional searches. That’s where AI Overviews aren’t, and where paid still earns patients — nothing there has broken. If you’re weighing whether ads pull their weight at all, is Google Ads worth it for a clinic covers that separately.
- Double down on local signals. With “near me” now a pure local-pack game, your Google Business Profile, your reviews, and your local SEO matter more, not less.
- Build authority so the AI cites you on informational searches. Those clicks have moved into the summary; the way back in is to be the source the summary trusts. That work pays off across Google’s AI Overviews and every other AI answer engine.
- Ignore the “ads inside AI Overviews” hype for now. It’s gated for healthcare and unproven for local. Revisit it when it opens up.
The short version: AI Overviews changed search, but for a local clinic they mostly rearranged the informational furniture and left your booking lane alone. Keep advertising where patients decide, and earn citations where they research.
In short
AI Overviews now dominate informational health searches but have been pulled out of local, “near me” provider searches — so a physical therapy clinic’s Google Ads and Business Profile still do their job on the transactional queries that book patients. The real shift is a split: advertise and stay locally visible on booking searches, and earn authority so AI cites you on informational ones. Ads within AI Overviews remain gated for healthcare, so there’s nothing there to chase yet.

FAQ
Are AI Overviews hurting my clinic’s Google Ads?
On your most valuable searches, no. Google removed AI Overviews from local “near me” provider searches — where your ads and Business Profile convert. AI Overviews mainly affect informational searches, which were research traffic rather than bookings.
Do AI Overviews appear for “physical therapy near me”?
No. Local, provider-finding searches now return the traditional local pack, listings, and ads. Google tested AI Overviews on “near me” queries and then pulled them back out.
Can my clinic advertise inside an AI Overview?
Not really, yet. Healthcare is kept out of the ads shown within AI Overviews, with only a small US test underway in AI Mode. Your standard Google Ads still run normally on other searches.
What should my clinic do about AI Overviews right now?
Keep advertising on local and transactional searches, strengthen your local signals and Business Profile, and build authoritative content so AI Overviews cite you on informational queries. See getting found in AI search.
